DEFINITION: Under direction, performs office and field work necessary for providing information and referral and social services for older people according to established standards; does related work as required. Performs professional casework including completion of care plans and social investigation involving recording and assessment of significant facts, drawing sound conclusions upon which appropriate action is recommended on behalf of clients, to clients, families, and related public and private agencies. Incorporates conclusions, recommendations and pertinent facts in documentation, reports, care plans, and correspondence. Maintains essential records. Provides advocacy on behalf of the consumer including creating, obtaining, or brokering needed consumer resources.
EDUCATION REQUIREMENTS: Bachelor’s or Master’s degree in social services or a health related field from an accredited college or university, a Bachelor’s degree in Social Work or State of New Jersey Social Worker Certification is preferred. One year’s experience in an agency operating program in work involving counseling, or assisting elderly with matters involving their welfare which can be substituted with a Master’s degree. Good driving record and possession of a New Jersey’s driver’s license. Sincere desire to work with disabled elderly and understanding of their many physical, social, and psychological needs.
HOURS: Monday through Friday, 20 hours per week
LICENSE: Appointees will be required to possess a driver's license valid in New Jersey
